Charlie, 
In NY we have teams working on reducing central line infections and are
submitting that data through our participation in collaboratives being
led by CHCA and NICHQ. Line infection frequency is trending downward in
our intensive care units and the improvement work now needs to be spread
to other units where central catheters are placed and maintained.  The
methodology in this campaign of observing for adherence to bundles of
processes and then aiming for 100% compliance for the whole process is,
I think, a good model for pursuing improvement.  Our adult units are
also specifically participating in a new york city wide collaborative on
CLIs. I am not sure whether their data is going to IHI but I think it
is; if the peds data is going it would be through CHCA, we are not
directly submitting it.
 
We are also doing work in the area of reducing adverse drug events,
focused on opiate management, and again in collaboration with CHCA and
NICHQ. This work has proven to be more difficult for a number of
reasons. Identifying what is a drug related AE is not always clear and
most of the AEs we are seeing have tended not to be terribly dramatic in
severity, so it is somewhat more difficult to get people's attention to
the importance of more standard med management using these cases as the
'burning platform.'  I think in 2006 as we all, in hospitals, must
further address the national patient safety goal/JCAHO standards
regarding med reconciliation at admission as well as through the
hospitalization and discharge, we will have to broaden this effort. I
think this plank of the campaign is very challenging because of the
frequency of the process (ie-choosing, ordering, delivering and
administering a drug happens tens of thousands of times each day), and
the relative infrequency of serious harm events. Of course, I do not
mean to minimize the fact that harm occurs, and occurs more frequently
than it should or that we should accept. But as systems we will struggle
with this because of the incredible number of people doing this
ubiquitous process that varies in detail on so many levels. Lots of work
still to be done here.
 
We are not addressing vent associated pneumonia, rapid response teams or
MIs. I expect that our work in terms of reducing surgical site
infections will ramp up in the coming year with our pediatric surgeons
really being tremendous leaders in our overall drive towards greater
consistency in our processes and decision making.
