We are a small
hospital with an outpatient infusion room. We do everything from port
flushes, lab work, IV antibiotics, PICC insertion and chemotherapy to mention a
few. We ar part of a larger system that has an in hospital infusion room
and an infusion room at an outpatient center. We recently worked on
combining all our policies and paper work so it would be the same. We came
up with new flow sheets and assessment forms but have found we need to tweek
them. We were told to work with them for 6 months and then we would meet
again to work on the flaws. If anyone has any forms they are willing to
