In our fluids infusing thru a PICC in the NICU, we put  0.25units hep/ml
if fluids are running at > 100ml/kg/day. If the total fluids are <
100ml/kg/day we add 0.5units hep/ml. No heparin is added to tunneled and
non-tunneled CVC fluids.

Dear Colleagues -
We are surveying other children's hospitals to see what their practice
is regarding central line maintenance with heparin infusions & flushes
in premies/newborns. Our preprinted orders for neonatal PICC placement
and maintenance states to add 0.5 unit/ml heparin to all IV solutions
that infuse at less than 10 ml/hour. This would deliver up to ~5
units/hour, not counting other sources of heparin, e.g., flushes, etc.
We are investigating the safety of these "low" doses of heparin in
premies/newborns, in particular with regard to whether this could result
in systemic anticoagulation (or otherwise alter coagulation parameters).

 
If you are adding heparin to your central line infusions, have you
measured PTT pre- and post-initiation of heparin?
