Currently our ID docs like a trough to be between 15 and 20 as long as the BUN and Cr look good.  Everything I'm hearing is the 5-10 is "old school" and ID now recommends much higher levels, however the literature hasn't caught up with the times I guess!  Have a difficult time convincing other docs to leave doses alone with troughs of 13 or 14.

Subject: vancomycin levels

I know this topic came through recently but what is everyone using for okay peaks and troughs with vancomycin—it seems like they were running higher and being okay.  Right now our normals are 30-40 for a peak and 5-10 for a trough.  I have a patient with chronic infection with a 39 peak and 10.7 trough and I’m not sure we want it any lower.  BUN and serum creatnine are really good.  RaKay
