We have performed this same program recently.

Alcohol prep pad for 15 seconds with friction and complete contact with top and 
treads of the cap.

The staff nurses were advised to sing - not out loud - row-row-row your boat 
... x 3.

 

Then after that in-service was done, I asked in private what the policy was and 
for the RN to demonstrate.

What an eye opener!  If they knew they were to scrub for 15 sec, they would do 
it for 3-5.  They were surprised they were so short of time.  And the majority 
of the time they pulled the alcohol out of the package and contaminated it - 
grossly- before using it.

After this hands on education, the infection rate went down substantially.

I have been asked to re-educate staff on the basics of prepping injection 
caps/sideports prior to access. The question has come up if there is a standard 
amount of time that is desirable (and realistic) for the prep. Such as alcohol
