Sorry I don't have a diagram for you but I commend you for implementing this. Sue Masoorli, legal consultant, emphasized at the recent AVA presentation, the need for all nurses inserting IVs to document the name of the vein, be it a peripheral or central line. "(R) lower forearm" is not acceptable. It is the nurse's responsibility to be aware of the venous anatomy & document such. (Thanks Sue- very informative session!)
