Absolutely! The INS standards of practice specially states that
the number and sites of all attempts should be documented. If there
has been multiple attempts with vein damage, the infusion of
medications distal to those sites could cause a serious infiltration
or extravasation injury. So this information is vital to figuring out
what happened for your sentinel event processes and for the expert
witnesses if there is a lawsuit. Lynn
