Vesicants are not always clearly identified. I can easily call vanc a
vesicant, because we have seen the necrosis caused by extravasation of vanc. It
may be rare, but it happens. I have also seen one pt with multiple areas of
necrosis from peripheral 5FU, at a time when 5FU was not yet being identified as
a vesicant. Every time the RN pulled out the PIV, the pt had necrosis at the
site. Apparently they were not flushing it properly prior to removal of the
cannula. The point is, there is no "Vesicant Idenitification Think Tank," where
all decisions about the identification of vesicants are made. Sometimes we have
to wait for bad things to happen before this determination is official.
