To expand on WVAs comment--If I understood MD presentation at Savannah AVA which makes science sense this is probably how PICCs become suspect in CRBSI--Infected Thrombus--So while the catheter may culture negative it is responsible for starting the infection cascade via thrombus formation--which begins at the moment of PICC insertion

You might try getting a Doppler study. With PICC's if IV fluid is leaking form the entry site, there may be a thrombus present.
