This has worked very well for our patients - they can manage to take it on and off with one hand and it really protects the catheter from accidental tugs that might result in displacement

A nice "home made" sleeve to contain the PICC extension is a sock.  Cut the foot off of a athletic sock and use the elastic to hold the extension in place, When the patient needs to access the PICC and tubing, he simply can push the sleeve up, connect the med and pull the sock sleeve back over, holing everything secure and in place.
