We require a specific order from the physician for a midline placement. (This covers procedure/treatment of a patient as well as reimbursement issues)    As it is a long peripheral cath with the full catheter length remaining peripheral, it does not require a formal informed consent just for that IV.  We do education with the patient and family before insertion and have an informal verbal consent at that time.
2. How are any of you working in outpatient ambulatory infusion centers obtaining Informed Consent for Blood transfusions (including a physcian signature)?
Our outpatient areas receive an order from the physician, the physician should have already discussed the transfusion with the patient, we then have the patietn sign the consent form upon arrival.
