Title: telemetry and PICCs
Well, sounds like I definitley need to reconsider our process for monitoring pts. 
 Do you take pre and post vital signs
as well?

I know in the Bard book it recommends cardiac monitoring during insertion of PICC line. 
 
 During our training
I asked our preceptor and she never did it.  We didnt do it then because of her.  I was thinking , the goal was
to maybe be able to assess any cardiac irritability if tip is placed in atrium.  I have never had anyone c/o palpitations or noted anything unusual even on the patients that we had tip found to be in the atrium. 
 
we dont even take vital signs before inserting the PICC line.

I just had a visit from our infection control MD. She states that the physicians/surgeons group have decided that they need to do telemetry when placing central lines and want to require it for PICC placements as well. I have one article from Pediatric Anesthesia that talks about V-tach with PEDS PICC placements but other than that, this is not something I have heard of doing on everyone. I told her my concerns with added cost, competency and having portable telemetry units available. Are any of you doing telemetry during PICC placements? I think this is overkill but I need all the info I can get before they act on this.
