We have a protocol that went through our committees that enables us to use 
bacteriostatic saline or topical cream.  Thus an order is not needed with every 
poke

the other question is 'when is "OK" not to use the
cream'? -- emergent situation, known allergy to lido?
some staffs claim that the cream(s) <might> cause
vasoconstriction. per my observation, the
vasoconstriction reaction on the same person is not
consistent.
