We will replace two lines that have be pulled out. Our Risk Management committee has decided that for the patient's safety, to place the third PICC requires an MD order for restraints and/or the presence of a "sitter". Any more than that and the MDs are encouraged to think of a different access device that is less delicate (hence less prone to fracture/catheter embolus) and less likely to be interfered with by the patient e.g. placement of tunneled catheter, Port, or jugular/subclavian CVC. It goes without saying that the PICC is securely wrapped with Kerlex bandage as soon as we identify the patient at risk of "Twiddler's Syndrome".

Question for the group. How many times would you re-insert a PICC line in a resident that keeps pulling his line out? He has a history of pulling 5 lines in the last week and a half. He is confused (obviously) but he thinks they are locks that are broken and must be fixed. -- Randy Ross R.N., B.S.N. IV Nurse Consultant, President & C.E.O. IV's Etc...
