Title: Bolus medications

What would be the reason for not giving the bolus from the bag that is hanging?  When I worked in a hospital (way back when) I would always use the pump to program a bolus dose, give a specified volume over a specified time, or some pumps just had a “bolus” rate already set,  so the nurse would just add the volume amount.  Then return to the primary infusion  when the bolus is completed.

Could you please share your practice for administering a bolus dose of a medication that the patient is already receiving as a continous infusion.  For example, heparin or insulin infusions - do you:

1.  Draw the bolus dose up out of a vial and give IVP?
2.  Draw the bolus dose out of the infusion bag and give IVP?
3.  Program the infusion pump to give the proper number of milliliters for the bolus dose?
4.  Request bolus dose from Pharmacy and give as a piggy-back or IVP?
