I just returned from a forms committee meeting seeking approval of our revised PICC insertion form which has on it that we obtained verbal consent, used Xylocaine, and an order for CXR, how to draw blood, (among other  things) which we place in the physician order section of the chart and a second form that describes the procedure, where the PICC is inserted,confirmation of placement, and dsg. change on it etc. This form is placed in the progress note section of the chart. The Committee did not approve these forms and would like me to consolidate these 2 forms into one form, due to duplication of information . Before I tackle this new project I thought I would ask what the rest of you are using. Perhaps one of you would be willing to share your forms. They also want to have a written consent form, which I have wondered about from the beginning.  Appreciate any help.
