This is not the most appropriate position but neither is the midline location. The NANN Guidelines for PICCs takes most of their malposition information from the adult literature and states "---use of the catheter should be discontinued if its position is not appropriate. To avoid vascular injury, the line should be infused with fluids that can be given in a PICC until it is properly positioned." Is there no way to work with a radiology group to do an overwire exchange or put in a properly positioned catheter?
