Joyce gave you a great hyperlink to about 7 Green Caddis patterns. The one I have in my database is:

Size - #10
Thread - black 6/0
Weight - none
Tail - none
Body - florescent green antron dubbing
Head - Rainy's sparkle dub in Emerald
It's tied like a simple larvae pattern with a tapered body. The head has a lot of loose fibers extending over the body.
