I never thread a worm on a hook.  I always use a treble hook and skewer him on all 3 points.  I also use a snap swivel, so I can undo it and skewer the worm over the eye of the hook too, then snap the swivel back on.   Tough to loose a worm this way.

Threading a Worm

Keep a little dry sand in your pocket, putting a little on finger and thumb will greatly improve threading the worm. An especially lively worm can be rendered temporarily docile by smacking it between the palms of the hands.
