I guess I've been fortunate, or else the rivers I've waded haven't been too rough, but I've never fallen- yet.  I have fallen getting to the river in some rough country, banging up some bones and gear, though.  Now I choose to float rivers, rather than wade, and I can't wait to try the Mokai out this summer on Wyoming/Montana back-country rivers.
