Most teaching / public hospitals are chartered as non profit organizations, they can't turn away a patient without insurance. They all have a fund for cover the costs of those patients, and some have special funding for certain tests and proceedures. Also, remember that if you are a cash customer, all fees are negotiable. Lot's of folks operate on a cash only basis quite successfully lowering the fees by 50% and more.
There are certain occupations where the person is considered self employed, for example commercial fishermen have a very difficult time getting both health and life insurance, if they can afford the premiums. So take care of health issues, don't sell a thing, and treat the 'patient rep' well, they can get a better fee, and a lot less down.
