I applied the 2.6-t4 patch, and all works almost perfectly, except I just
dialed home to check that it recorded a show properly, and it didn't.

Subsequent testing reveals transcode (with options -p /dev/dsp0 -i
/dev/video) gives errors:
ioctl (VIDIOCGMBUF): Invalid argument
error grab init
video import module error: OPEN failed
[transcode] critical: failed to open input source

xawtv works fine.

Any obvious signs of stupidness on my part?
Nothing in the logs saying anything intersting, but I haven't yet tried
with debug options (for some bizaarse reason, I don't want to reboot my
machine again, over the modem - heh, fun)

I guess it's a missing

req.memory = V4L2_MEMORY_MMAP;

line in transcode source, see my previous post here. Try to report your problem to transcode developers.
