You can do either. In the latest version (4.1), The default profiles are small, medium, and large, but you can tweak any of them (as I usually do, to suit each particular video), or roll your own. One thing that was better in Sorenson 3.x is that it used to give an on-the-fly estimate of the resulting file size every time you changed a setting. This doesn't work in the current version, someday I have to make the effort to see if that's a bug that they've fixed...
