Ok, I was playing devil's advocate a bit.

I've learned from years of dealing with this stuff:

You can't trust the file extension.
You can't trust the mime-type.

Sometimes it's a mis-configured server, sometimes it's a clueless user, 
sometimes is malicious. (Usually the first two.)
