Look at as many releases as you can. Talent and extras should be paid at least a dollar ($1.00). What you end up using the video for will make a lot of difference in the selection and legal language that must be included in a release form.
Using Google search: talent + release + form extras + release + form A Best example to give you a solid foundation is located at The 48 hour film site <http:// www.48hourfilm.com> <http://www.48hourfilm.com/filmmakers/documents.php> <http://www.48hourfilm.com/filmmakers/48HFP.TalentRelease.pdf> Tip, Hint, Trick: Use Safari on Mac to view the page or if you have gmail, email the PDF to yourself and preview in Google Mail. PAY special attention to paragraph 3. or the BBC clause! (BBC means Big Butt Cover) which will keep your A$$ out of a sling today, and in the unimagined, not yet invented future. I did not see it mentioned but it is good practice to have copies of ID card or drivers license attached to your contracts. Rolling video of the person signing the contract or having the person say their name, age (or date of birth), and social security number prior too starting the interviw is redundant but good practice. It is important to PAY attention to everything once you PAY attention to one. A location you shoot at/in, the person or business may be renting or leasing the location from someone else. You need the permission of the "Owner" of the property in most cases (unless the Owner grants the renter rights to enter contracts - always confirm). Use the record rooms of the city or town you are shooting to check ownership of a property if in doubt. --- In [email protected], "Jay dedman" <[EMAIL PROTECTED]> wrote: > > Does anyone here use a Release form when you interview people? > Something that basically says, "you allow me to use your image in my work". > > I'm looking for a form that is human readable...and not full of legalese. > Ill write one myself, but I assume someone must have thought of this before? > > Jay > > -- > Here I am.... > http://jaydedman.com > 917 371 6790 > > Check out the latest project: http://politicalvideo.org > 500 hours of George Bush speeches > Search, download, use >
