Kathleen,
There are a number of good resources online to find sample release forms or
even templates to use. For example:
http://www.videouniversity.com/articles/releases-for-use-in-film-and-video / or
http://www.digitalgov.gov/2013/05/04/video-copyright /

Hi all -

Do any of you that run media shops that do a lot of production support have
templates for release forms that you share with students or other community
members that may need them for projects, especially for interview subjects?

Thanks so much!

VIDEOLIB is intended to encourage the broad and lively discussion of issues
relating to the selection, evaluation, acquisition, bibliographic control,
preservation, and use of current and evolving video formats in libraries and
related institutions. It is hoped that the list will serve as an effective
working tool for video librarians, as well as a channel of communication
between libraries, educational institutions, and video producers and
distributors.
