":map" creates a mapping for Normal, Visual and Operator-pending (not Insert)
":map!" (with bang) creates a mapping for Insert and Command-line (not Normal)
Why? Almost every command differs between N/V/O and I/C. OTOH many commands
(including e.g. |i_CTRL-V_digit| or |i_CTRL-R|) are shared between I and C.
Similarly, ":map" with no arguments displays the mapping for N V and O,
":map!" with bang but no arguments displays those for I and C.
