Most helpfiles are in ASCII anyway (which is compatible both with UTF-8 multibyte and with ISO-8859 8bit). A few, like arabic.txt, are in UTF-8 but won't be needed when compiled with -multi_byte. options.txt is in UTF-8 because of the example for 'langmap' which includes Greek letters, and is clearly marked as being in UTF-8. (By default, +langmap is compiled into Big and Huge builds only, while +multi_byte is already in Normal builds of Vim.)
For translated help, the _helpfiles_ may be in UTF-8 for non-ASCII characters in the languages concerned, but IMHO the _tags_ themselves ought to remain identical with those in the English-language help, i.e., untranslated and in ASCII, so they will be found regardless of which help language is in use, and even if one translated helpfile is missing (in which case Vim falls back on the English help).
