I'm not following you. With 'spell' on, Vim checks for spelling errors in all the words in the document all the time as changes are made. That functionality is needed for ]s et al. to work. On the other hand, z= is asking Vim to check just one word, right now, to see if it's spelled correctly. The :spellrepall command just repeats the correction operation, like an ordinary search and replace, on all the other occurrences of the misspelled word.
