I'm not following you.

With 'spell' on, Vim checks for spelling errors in all the words in
the document all the time as changes are made.  That functionality
is needed for ]s et al. to work.

On the other hand, z= is asking Vim to check just one word, right
now, to see if it's spelled correctly.  The :spellrepall command
just repeats the correction operation, like an ordinary search and
replace, on all the other occurrences of the misspelled word.
Neither of these require Vim to highlight anything or automatically
check words as you type.
