runtime(filetype): ft detection maybe wrong if 'fic' set for *.[CH]

runtime(filetype): ft detection maybe wrong if 'fic' set for *.[CH]
So only set the filetype to cpp for *.C and *.H files, if
'fileignorecase' option is not set.
