1) There is the "Edit in MacVim" hack but it does not work in 10.6
unless you run Mail in 32 bit mode.  You may want to check out
QuickCursor [1] if you're on 10.6.

2) Yes, but its slightly complicated:
  * Open Xcode preferences
  * Select the "File Types" tab (you'll have to scroll to the right)
  * Expand the "text" item
  * Next to the "sourcecode" item click on "Default (Source Code
File)" in the "Preferred Editor" column and choose "External Editor ->
Other ..."
  * Browse to MacVim and select it

The Xcode integration is not very good though (due to limitations in
Xcode).  The only thing it does is to open MacVim if you double-click
on a source file or a build error.  I hardly ever use it.
