I think the problem with the conceal patch is that it is so large: it modifies lots of code in lots of different places. Therefore I imagine Bram would be concerned that unexpected side effects and/or bugs might arise if it were incorporated into Vim. Even as a compile-time option (which is how it is structured), the fact that it inserts code into many places raises the complexity of the source a fair amount.
