It seems like your runtime files are corrupt somehow.  Try downloading
the latest version of MacVim and overwrite your current version.  If
that doesn't fix it, then you must have installed some plugin that is
removing menu items.  You'll have to figure out yourself which plugin
this is (by disabling your plugins and then enabling them one at a
time).
