Ctrl+Tab is, unfortunately, difficult to bind, as tab in vim is == Ctrl+I. 
I agree that this would be a nice thing for MacVim to fix, since there's no 
reason I can think of to let the Ctrl+Tab keystroke through to vim.

As far as automatically switching directions, that sounds like a terrible 
solution to a legitimate problem (see also: 
http://www.azarask.in/blog/post/solving-the-alt-tab-problem/ which has a 
"solution" that I also don't like). If you really want it though, it could 
probably be done by remapping gt. You could also remap tg, but that's 
already something else (move 't'ill the character 'g' on the current line, 
i.e., one character in front of it).
