Otherwise three-in-a-row looks good. Everything depends on what his/her mailer says about the bounce. Then again, sometimes the mailer lies: I've seen errors like "account doesn't exist anymore" for transient errors, and the message was ultimately delivered. In other mailing lists I've managed I've applied what I consider a very flexible solution, but requires some manual work: the first bounce is ignored, everyone has a bad day, but the date is registered. If more bounces happen immediately the offender is forbidden to post until the issue is resolved. Otherwise, if additional bounces happen after a week or more, a private message is sent to clarify the issue (transient errors, account changes, whatever). If the private message bounces too, another week and another private message. After the second private message, a week of grace-period for the answer, no matter the additional bounces. After that, unsubscribe. I'm sure there are much better schemes out there (Mailman has one, for example), and I confess I haven't put the above into practice more than a couple of times, so...
