_Starting_ Visual or Select mode by dragging the mouse's first (usu. 
left) button happens _from_ Normal or Insert mode, so it's related to 
the n and i flags of the 'mouse' option. _Extending_ an existing 
selection happens by clicking the second (usu. right) mouse button if 
'mouse' includes v or a and 'mousemodel' is "extend", or the first 
button with Shift if 'mouse' includes v or a and 'mousemodel' is "popup" 
or "popup_setpos".

The 'mouse' flags always relate to what mode was active _just before_ 
you used the mouse. See also 'mousemodel' for what the mouse does in 
various circumstances.
