Looking at the markings, it seems to be a Novy Systems accelerator.

There's mention of a driver for one here: 
http://www.macdrivermuseum.net/accel.shtml

I can't say whether that card matches that driver, but it might :) 
There's also mention of a Quik30 that works with the Total Systems 
drivers. Can't say I've messed with any of it, someone more familiar 
with it might be able to tell you more about the card itself.
