2. The following (from my notes) might be of some help to you...

How to format a System Boot Disk for a classic Mac (from within a
Windows environment):

a) Download the Network Access Disk image at
http://download.info.apple.com/Apple_Support_Area/Apple_Software_Updates/English-North_American/Macintosh/Utilities/Network_Access_Disk_7.5.sea.bin
b) Use Aladdin's StuffIt Expander (Windows edition) to extract the
disk image (look for a *.image file inside the unarchived folder)
c) Insert your blank floppy (into a PC or USB floppy drive if you're
running OS X), and format it as FAT (DOS) format (this is important to
avoid later unreadable sector complaints)
d) Download, install, and start up WinImage
e) Open the *.image Network Access Disk image in WinImage
f) Select Disk > Use drive B: (or whatever drive your [USB] DOS-
formatted floppy is in)
g) Select Disk > Write Disk
h) It may say something like "format is not correct, do you want to
reformat?" if so select Yes (or Format)
g) It may take a minute or two, but the image should then be written
to your floppy in HFS format
h) You can open your floppy in HFVExplorer (if you've downloaded that
separately) to confirm that the disk contains the system files needed
i) Remove the floppy and try booting from it in your classic Mac.
Worked for me!

It took me a while to get this process sorted out myself, but
hopefully it will save you some time.
