Try this utility to format the drive.

http://www.jmug.org/software/thepickle/Archive/Mac_Utilities/SCSI_director.img.sit

You'll have to unstuff it on a Mac then use Disk Copy
(free from Apple) to make a floppy.

OR if you have a PC you can download Stuffit Expander
for Windows, which is freeware, unstuff the image then
use RAWRITE.EXE from:

ftp://ftp.slackware.org/pub/mirrors/slackware/slackware-8.0/bootdsks.144/

to write the image to a floppy. It even works in a
DOS or command prompt.

I think that might even be a bootable disk so you
can just pop it in and go.
