One thought might be to use GoldWave on your PC and set a mark at each tone index, then split the book into separate files at those marks. Then, you could edit the tag information for each file so they would be treated as one album. Then, when you bring it into iTunes, you could further edit the tag information and change the kind to Audio Book. Then, each page (or whatever the tone indexing was) will be its own file. GoldWave has a fully functional demo, but is only about $50 to purchase. www.goldwave.com The manual is accessed off the help menu and is in html so very easy to navigate. Virtually everything can be done form the keyboard for the editing functions. I suspect there is similar audio editing programs on the Mac side, but I haven't used any of them.
This would all be very time consuming, but it might get you what you want.
