Yes I took the liberty of contacting Apple Accessibility about this problem.  
You are correct both work arounds are effective for now and this is a known 
issue for which they requested of me to provide log reports for further 
investigation.  so they are aware of it and are attempting to get to the root 
of its causation.
Yay Apple Accessibility.
It always pays to contact and report things never assuming that anyone else has 
and if they have the more voices reporting issues the more impactful. that’s my 
view anyway.

Hello. I am using a 4S with the latest firmware, and I have experienced this 
problem twice. Both times, I tried turning sounds off, then back on, and the 
voiceover sounds were still gone. The first one was corrected with a reboot. 
The next time it happened, I tried turning voiceover off and back on, and that 
worked.

  Robin, this is what I was referring to a couple of days ago. I will do 
nothing different and suddenly some of the sounds that indicate opening, 
closing, of an app or reply menu in email will be gone. I can find nothing to 
start it and I have no control when the sounds reappear. I have noted, however, 
that a reboot may but may not help. That is why I asked if Apple had control of 
the sounds. I did not believe it, but it almost seems that way. I have an 
iPhone 6 running the newest update.
