Hi, Yes I took the liberty of contacting Apple Accessibility about this problem. You are correct both work arounds are effective for now and this is a known issue for which they requested of me to provide log reports for further investigation. so they are aware of it and are attempting to get to the root of its causation. Yay Apple Accessibility. It always pays to contact and report things never assuming that anyone else has and if they have the more voices reporting issues the more impactful. that’s my view anyway.

I am using a 4S with the latest firmware, and I have experienced this problem twice. Both times, I tried turning sounds off, then back on, and the voiceover sounds were still gone.
