In case it hasn't been brought up yet, I wanted to mention that this feature doesn't require Siri. That works, of course, and I never knew about it, so thanks Jonathan for pointing that out. To start the command with just a gesture, though, you can touch the Status Bar and then perform the two-finger swipe down. Just as three-finger swipes act differently in the Status Bar, so does this command - it starts Speak Screen.
I believe it will also work with headphone remotes, though I haven't tested this personally yet. Another thing I am pretty sure will work is using a braille display or bluetooth keyboard: go to the Status Bar, then do the equivalent of a two-finger swipe down (such as a space-r). Again, not tested, but I don't see why it wouldn't work. It would be interesting to hear from someone who can give it a try. Quite handy for reading Instapaper or Pocket articles, among many other uses.
