Yes, While I mentioned Access Note, I do agree with Richard about Voice Dream Writer. It is accessible, and one can mark up, or in this case, mark down, text files and export them to a number of formats including HTML, where headers that are marked up in the text are honored by VoiceOver and other screen readers. This is really nice for being able to jump around in a file and find exactly what you want.
