I saw this update yesterday as well. The only thing I'm noticing and perhaps others can confirm whether they're experiencing it is when first launching the app and while bluetooth headphones are pared, Voiceover has the sound of that microphone bug from earlier iOS versions. It sounds like you're on a call or engaged in the dictation feature. Pressing and holding the home button as though you're going to talk to Siri and letting go once the chimes sound will make it go away though.
The other thing and this is more an iOS issue is that even though I only have iOS Spanish Mexico loaded on my iPhone and selected as one of my languages in the language rotor option, Voiceover will speak the Spanish language translation in Spanish Spain.
