The headphone and speaker have separate volume controls. With the headphoones 
plugged in, turn the volume up. Once you set these controls, they won't be 
modified unless you change them. Whatever is sounding will change when you 
press the volume controls. One exception to this, depending on how it's set, is 
the "change with volume controls" option in sounds. If this is off, your alert 
tones and alarms will not change with the volume controls.

Hope this helps. It can be confusing at times, but I do find Apple's 
implementation of volume controls to be quite clever.
