You can place it flat on a table or desk, and type with your fingers in a kind of flattened V shape (on iPads and larger iPhones, you can use more of a straight line shape).
For on-the-go typing, I hold my phone against my abdomen, gently squeezing the edges with my pinkies and thumbs. This is known as "away" mode. In this method, your six fingers still do the typing, but they are oriented with the short edges of the phone. Your right three fingers are near the Home button, your left near the camera. It takes some practice to find the optimal position for you, but it's pretty effective once you get it. Remember that, in iOS8 and later, Apple offers built-in braille typing. Add it in VoiceOver's rotor settings, then simply rotor right once whenever a keyboard is on screen.
