When I changed the setting to first name and last initial, it did not take effect immediately. So I powered the phone off and back on again. Now the setting change took effect.
The only complaint I have is that someone whose last name begins with "V" is spoken as "fifth" as in Michael Fifth. But that is not a big deal.
