Thanks for this. Question, if it's set permanently to head set then does 
that mean I need to use ear phones with it?

Sounds like a proximity sensor problem, where the sensor is sometimes 
thinking your phone is held to your ear and sometimes thinking it isn't. I 
suppose a good cleaning (be sure to look up how to properly clean touch 
screens) couldn't hurt, particularly around the sensors. In the meantime, 
you can go to Settings > General > Accessibility, and choose the Call 
Routing option. Set this to whichever you want; you can still use the 
speakerphone, but you'll have to manually enable it instead of letting it 
switch automatically.
