Hi Judy, I've seen and still experience both of these from time to time.
I find both an annoyance but, with the first, I just put up with it as it happens even when I am not using a keyboard with the 4S. With the second I was reliably informed that you can in fact remove Voice-Over from the phone with a tripple click of the Home button, then put it back in again. It really does work, presuming you've got it set up to turn on/off Voice-Over in this way.
