For whatever reason, and I hope this stays this way, subsequent connection and 
disconnection of my iPhone with my Amazon echo via Bluetooth did not result in 
volume changes. The phone and voice over volume did not go to zero when I 
connected and disconnected. I have no idea why it messed up both ways the first 
time, but it worked right the next time I connected and disconnected. I am 
using an iPhone 5S, if that matters. I have not yet tried my old iPhone 4s or 
my iPad mini.
